About the Black Health and Social Services Hub

The Black Health and Social Services (BHSS) Hub offers a range of Afrocentric programs and activities in primary care, mental health and addictions, and social services. Roots Community Services, LAMP Community Health Centre and the Canadian Mental Health Association (CMHA) Peel Dufferin are partnering to offer this initiative which is being designed with the holistic health of the individual as the primary focus. This ensures that services are easily accessible and closer to home for Black, African and Caribbean (BAC) communities in Peel.

Our communities' input is invaluable as we continue to develop the BHSS Hub and we will be reaching out regularly to community members to gain insights and obtain feedback.

We are continually working to evolve our programs and services to meet the diverse needs of BAC communities in Peel region.

Primary Care

Through a team of family physicians and nurse practitioners, we support the communities health care needs with services that include: care for children, prenatal care, minor office procedures, gynaecological care, adult health maintenance, and chronic disease management (i.e. diabetes). With a strong emphasis on health education and prevention, care in provided with the client’s needs and health goals as our priority. 

The main goal of the Diabetes Education Program is to improve the quality of life of clients 18 years and older living with prediabetes, Type 2 Diabetes, and those at risk of developing Type 2 Diabetes.  Services are offered through one-on-one counseling and group programs.

One-On-One Counseling:
Our skilled staff teams consisting of a Registered Nurse and a Registered Dietitian will meet with clients one-on-one to provide free diabetes prevention and self-management services, in a supportive and inclusive environment.  A care plan will be developed that focuses on the client’s needs, available resources, and existing support networks, in order to put the client first.

Group Programs:
Our group education sessions are designed to provide practical information on diabetes self-management. It is our hope that these sessions will help clients to understand diabetes with support, practical tools, and guidance.  Group Sessions covering the following topics:

  • Introduction to Type 2 Diabetes

  • Understanding oral medications and insulin

  • Tips for Meal Planning, Grocery Shopping, and Reading Nutrition Labels

  • Healthy Eating and Exercise

  • Stress Management and Emotional Eating

  • Physical Activity

  • Diabetes and Healthy Feet

 

Eligibility Criteria:

  • One-on-one counseling: Clients who are diagnosed with prediabetes, Type 2 Diabetes, and at risk of developing Type 2 Diabetes.

  • Group programs: same as above, with flexibility to include those who may not have diagnoses

Mental Health and Addictions

Supportive services for clients that may be dealing with mental illness, substance use or have behavioural addictions concerns. We provide an environment where clients can have their concerns heard, and we work to develop a plan of action that uses a combination of harm reductions, abstinence-based strategies along with short-term counseling and case management support to ensure progress toward client goals. We also provide education on subtance use and wellness to client and their families/other supports. 

Help for clients with symptoms of mental illness an emotional difficulties. Using a “brief counseling model” we offer an immediate opportunity for therapeutic conversation within a short period of time. Our aim is to focus on the immediate needs and strengths of clients to work towards actionable solutions and creating a plan. 

Our case management services help provide ongoing support for clients with serious and long-standing mental health concerns. We work with clients to identify their needs, develop a care plan and support and monitor clients as they work through the care plan strategies. We focus on small actions that clients can take everyday to work towards health in all areas of life. Our clients care plan are developed in collaboration with a multidisciplinary team of professionals, committed to their health and wellbeing.

Multi-Disciplinary Team (MDT) The multi-disciplinary team provides culturally affirming, holistic integrated care for people from Black, African and Caribbean communities who are impacted by mental health and addictions. The Team is a collaborative group of professionals from various disciplines, who work together to deliver services that address an individual’s wellbeing while ensuring that approaches to care are informed by clients and are effective in meeting their needs. This team-based approach allows for seamless transitions between different services, allowing for the right care at the right time, by the right person. 

Assertive Community Treatment (ACT) is a team-based approach designed to provide comprehensive, community-based mental health supports and treatment for people living with serious and persistent mental illness who have not responded well to traditional outpatient mental health interventions.

ACT teams provide initial and ongoing assessments; holistic integrated case management supports including employment and housing assistance; family support and education; addiction services; and other services and supports critical to an individual’s recovery journey.

The ACT and EPI team consists of the following disciplines:

  • Psychiatry – Conduct assessments, provide treatment and support
  • Nurse – Provide health assessments and medication education and support
  • Community Support Worker – Offer supportive counselling, crisis intervention and case management
  • Peer Support Workers – Provide support from a lens of lived experiences
  • Family Worker – Offer psychoeducation, mental health system navigation, family mediation and supportive counselling
  • Addictions Support Worker – Provide assessment and treatment to address concerns with addiction
  • Social Worker – Conduct assessment, individual and group intervention and treatment plans, crisis intervention to aid clients in achieving their identified goals.

Early Psychosis Intervention (EPI) is provided in a multi-disciplinary team-based approach to care to individuals ages 14-35 who have experienced a first episode of psychosis to lead fulfilling, satisfying lives within their community.

The purpose of the program is to provide support in achieving their recovery goals, education, minimize disruption and societal impact by reintegrating them into the community and school, and develop coping strategies. The program also offers various groups including social outings, therapeutic groups and skills building.

The ACT and EPI team consists of the following disciplines:

  • Community Support Worker – Offer supportive counselling, crisis intervention and case management
  • Peer Support Workers – Provide support from a lens of lived experiences

Social Services

We are here to connect Black, African and Caribbean community members with supports that help you to navigate your path to wellness. We understand the needs of our communities and our support line offers a connection to providers who are attuned to the lived experiences of people from the African diaspora. Our confidential and non-judgemental team offers a culturally-responsive listening ear.  We provide pertinent information to help you find answers to your questions. We also assist with referrals to a wide range of resources including, counselors, community programs and services. 

Our Care Navigation and Wraparound Support service recognizes the importance of prevention in living a healthy lifestyle. We offer culturally-appropriate and adaptive health education that encourages BAC community members to engage in regular health checkups and screenings. We help our clients navigate the primary and mental health care systems with ease. Our Care Navigators are well equipped to provide culturally-affirming support to individuals, ensuring a seamless connection to relevant resources, services and support.

Our approachable, knowledgeable and skilled Community Ambassadors host outreach activities to reach deep into communities and connect with persons who need services most.  Our ambassadors provide education through various  types of events, community presentations and workshops. We aim to address health and wellness needs, improve health equity and rise above the stigma around mental health and addictions. Our community ambassadors are here to connect you with the wide range of programs and services available at the Hub, offering information with a culturally-affirming and supportive approach.
