One Year of Integrated Care at the BHSS Hub

This month, we highlight one year of integrated care at the Black Health and Social Services (BHSS) Hub; a milestone that reflects our shared commitment to community wellness. Through collaboration across primary care, mental health and addictions, and social services, our teams have built a model centred around people and culture in every aspect of care.

Integrated care at the Hub means working together to meet clients where they are. Our physicians, nurse practitioners, therapists, social workers, and community health navigators provide coordinated support that addresses the interconnection between physical, emotional, and social health.

Over the past year, our services have helped individuals and families access primary care, counselling, early psychosis intervention, and community programs in one connected space. Each visit is guided by compassion, trust, and cultural understanding.

This work is led by Roots Community Services and our partners: LAMP Community Health Centre, and the Canadian Mental Health Association Peel Dufferin. Together, we continue to expand access to care and strengthen the foundation for healthy, thriving communities.
